Welcome to the Local Government Compliance Certification

The following certification is required for prospective medical cannabis establishments.

To attach local ordinances to this certification, click on the paperclip in the upper left-hand corner of the form. Once completed, you will receive a copy of the completed form and it's attachements in your inbox. Businesses applying for a medical cannabis establishment certificate are requred to include the completed document along with the attachments when submitting their applications.
