Welcome to the DOR Order Form

This form allows you to order clothing options that will have the new DOR logo on the upper left section of the shirt / cardigan. Dark colored clothing will have the white logo; lighter colored clothing will have the black logo. Both logos are embroidered on the clothing.

The form automatically calculates the amount due. When you are ready to place the order, please check the box at the bottom of the page acknowledging that you understand that payments must be submitted before the order will be placed.

You will have a couple of options to pay. We are going to utilize the "Make a Payment" system that is on Department's website.

This site allows you to securely submit the payment either by providing your bank information (no extra fee) or a credit card (2.45% convenience fee). Once you have submitted the order you will receive an email that will provide the license number and owner name you will need to complete the transaction.
